I think the point the NTSB is making is that the tolerances for the separation of traffic at DCA are far too thin, meaning that even a minor deviation can cause a fatal conflict.
Can’t assume that everyone does it perfectly every time.

The near miss numbers at DCA are truly mind boggling.
The frequency of a helicopter and airliner being within 1nm laterally or 400 feet vertically is 13. A day!
This was fun.
More details to follow, but engine touble, flight diverted, yada yada yada engine fire at the gate so we exited off the other wing.
Everyone is ok.
Flying is no longer the new Greyhound; it’s the new Oregon Trail.
Glad you’re old. Hope you can make it over to the Centurion Lounge for a recovery libation.
I was gonna say, you made the news!
I resemble that remark.
By the time I was out there they had rolled a baggage conveyor over so I came down that.
Both the flight crew and the ground crew were very helpful.
Most passengers were calm enough.
Wow, I’m a dumbass: I failed to comprehend that was you reporting in the first person. My brain just interpreted it as a random tweet that was forwarded. Glad you got out of it ok!
I’m glad you’re OK. I’ve been trying to figure out whether it would be insensitive to mention this; everybody knows I’m a paragon of sensitivity. My son is what you might consider a connoisseur of aviation disasters. I find it troublingly macabre and voyeuristic, but he has amassed quite a broad knowledge of important aviation mishaps. Fortunately your experience was less of a disaster and more of a supreme inconvenience, but I’m sorry to say that my son would have enjoyed the experience thoroughly. Again, glad you’re OK.
Yikes, I’m getting old. Meant to say safe.
Did you get out your phone, grab your carry-on, and record your exit from the plane like a real goddamn American?
Give your emotional support animal a minute to get his shit together, and go back to emotionally supporting you.
Hey, SWA John? My cousin (American) said kinda-recently that a pigmy pig, and an iguana, were the weirdest ones…what have you seen?
Glad you are ok.
So glad no one was seriously injured or worse.
To conclude:
We stuck it out in Denver and finally took off (in a different plane, or so they claimed) at 0230 mountain time.
Walked into the house about 0615.
And no, I didn’t livestream it for the gram. But there were plenty of people with their phones out while standing on the wing.
I’ve seen an emotional support pig once, but not an iguana or snake, et al.
The gate lice/skin-bags/self-loading and unloading cargo/passengers are “interesting” to say the least
I’m so relieved and glad to hear you all are safe- I’m also relieved it wasn’t a messy egress- I worry about people losing their shit and making matters worse if there’s ever an evacuation.
